The undersigned Certified Contractor requests RWSI warranty review and material quote. Contractor hereby warrants compliance with RWSI published and/or written project requirements and applicable project documents, including manufacturer's specifications and recommended installation practices for the job described herein. Contractor further confirms reading and understanding the provisions and limitations of the written Project Prequalification Form and Warranty Guide, including sample forms and warranty. No review can be scheduled and no warranty review or issuance can occur until all warranty fees are paid in full. Contractor certifies all work, including punch lists, has been completed. Contractor understands that RMI is only providing material pricing and not material quantities which is the sole responsibility of the Contractor ordering materials.
